Completing the two steps below will submit all information needed for Jackson Girls' Lacrosse Athlete Registration.  In step 1, you will complete a fill-in form of the Emergency Medical Form.  When completed, please save it to your computer.  You will upload it to your Athlete Registration form (step 2).  Payment will be requested via paypal at the end of step 2.
